As medical costs continue to rise, it's important to have access to quality care at affordable prices. As a SHAZAM employee, you can choose between three levels of medical coverage. This allows you to choose a plan that fits your and your family's medical needs. All our medical plans offer a broad array of services, including hospital and physician services, prescription drugs, and preventive benefits. Coverage is available to you and your eligible dependents. Your pre-tax contribution varies with the medical plan you choose and your employment classification.
Dental Benefits
SHAZAM employees may select between three levels of dental care. Coverage ranges from preventative to restorative to prosthetic coverage, depending on the plan you select. Orthodontia coverage is available for employees and eligible dependents. Your pre-tax contribution varies with the dental plan you select and your employment classification.
Vision Benefits
As a SHAZAM employee, you may select between a fully insured vision plan through VSP where exams
and lenses are covered each year (in-network) and frame allowances are offered every other year.
This plan also offers discounts on scratch resistant and progressive lenses as well as Lasik
surgery. Employees also have the option to elect a vision program that offers discounts on your
eye exam, glasses, and contacts at the time of the sale. You must go to a participating provider
to receive the scheduled discounts.
Flexible Spending Accounts
Out-of-pocket medical expenses can add up quickly. A flexible spending account can help you defray these costs. SHAZAM's medical spending and dependent care assistance accounts allow you to make pre-tax contributions that you can use to be reimbursed for eligible healthcare or day care expenses you incur. You can participate in one or both accounts.
Fitness Reimbursement
After one year of service, SHAZAM employees may be reimbursed up to $300 per calendar year (taxable) for eligible fitness equipment and memberships. Eligible reimbursements are for stationary cardiovascular equipment or health club memberships that offer aerobic activity or stationary cardiovascular equipment.
